🚨 Important Medicaid Insurance Update

We want to inform all patients that UnitedHealthcare is no longer participating in Louisiana Medicaid plans.

📢 All Medicaid patients must check their insurance plan AND confirm their P*P is correct.

If you or your child currently have UnitedHealthcare Medicaid coverage, you must select a new Medicaid health plan as soon as possible to avoid any interruption in care.

✅ What you need to do:
• Choose a new Medicaid plan
• Bring your new insurance card to your next visit
• Make sure your Primary Care Provider (P*P) is updated to Dr. Lawanda Lamar Bellamy, MD

⚠️ If your plan and provider are not updated, it may delay or prevent services at your visit.
