Last season, we teamed up with the to celebrate 11-year-old Noah, an Aurora Children’s Health patient from Hartford who lives with type 1 diabetes. To mark his birthday, Noah was surprised at American Family Field, complete with Brewers mascots, team gear and a special behind-the-scenes experience with the team.

Tonight, Noah is back at the ballpark — this time in the Aurora Health Care bullpen — to root for Milwaukee as they take on the Dodgers in Game 1 of the National League Championship Series. 💙💛

We’re proud to cheer alongside him as the Crew™ continues their postseason run.

Kathryn, Nurse of the Year for Aurora Continuing Health, serves as a hospice RN at Aurora at Home Hospice in Racine and Kenosha. She effectively coordinates care in diverse settings, creating individualized plans that prioritize patient autonomy and dignity. Her strong assessment skills and collaborative nature enhance patient outcomes, and she is a fierce advocate for patient rights. She serves as a preceptor for new nurses, modeling excellence in symptom management and holistic care. Her thoughtful approach and effective communication make her a role model in hospice and end-of-life care, embodying the essence of compassion and integrity.

“My mother is a nurse, and she was always an inspiration for me while growing up. I always tagged along on ‘take-your-children-to-work day,’ and I could see the difference she made. It inspired me to do the same. My love for people and compassion to help others drew me to nursing. For hospice nursing specifically, I lost my dad when I was younger. The compassion we were shown by the medical staff is something I will never forget. I want to provide that for others going through the same thing.”

Across the state, our teammates are cheering on the Milwaukee Brewers as they continue their postseason run tonight in Game 4 of the NLDS. While the Crew™ continues to try to bring home wins, we’re celebrating one of our own.

"They aren’t just a patient — they’re someone’s parent, sibling, child, friend.”

Guadalupe, Community Health Worker at Aurora Walker’s Point Community Clinic, brings heart and humanity to every interaction. She’s committed to building trust, breaking down barriers and empowering people to take charge of their health.

Whether she’s delivering test kits, picking up results, following up with patients, providing accessible health education or even visiting local food pantries to connect those in her community with various resources, Guadalupe meets patients where they are with compassion and dignity.

“I’ve been through the same situation. I was a patient at this clinic before my role here and so was my mother. When we explain things in ways that are digestible and respectful, people feel empowered to take control of their health. We have to be their advocates. That’s how we build lasting bridges of trust.”

This year, Guadalupe was honored as a Community Health Worker Cancer Champion by the American Cancer Society and Kohl's Healthy Families, a recognition of her commitment and dedication. Serving over 1,000 patients annually, she connects clients with services that promote early screening and informed decision-making.

She calls her role one of the greatest blessings of her life. We call her a hero. Through empathy, advocacy and action, Guadalupe is saving lives. ❤️

We offer multilingual, culturally sensitive care, with no insurance required. Donations are greatly appreciated but never required. Taylor, Nurse of the Year at Aurora Medical Center – Sheboygan, serves as a lead RN in the Critical Care and Cardiac Unit. She effectively coordinates care for critically ill patients, ensuring efficient workload distribution and team support. As chair of the Cardiac and Critical Care Unit Professional Governance Council, she advocates for patient rights and pushes for improvements that benefit both patients and teammates. She mentors new nurses through their transition into practice. Her role as a Patient Safety Champion emphasizes her focus on maintaining high standards of care.

“The support and encouragement I received from my coworkers and managers over the years has shaped my professional growth. They all helped me believe in myself. This began from my first day as a CNA, my nurse extern days, as well as becoming a lead nurse and Professional Governance chair. Another big element that shaped me is my family. They taught me so many life skills that I consistently use in this field of work, and they've always been there.”
