Assertive Community Treatment (ACT) Team Co-Occurring Disorders Counselor

Archway Station, Inc. located in Cumberland, Allegany County, and Oakland, Garrett County, Maryland is a non-profit agency employing 150+ employees and serving the needs of individuals with behavioral health and substance use disorders.

The ACT team shares the responsibility to carry out the activities designed to assure ACT mission success on and off-site, in groups, and one-to-one contacts, in formal as well as informal situations. The ACT team has the primary responsibility for all members' treatment plans, shared responsibility for the ACT caseload, and shared responsibility to work out schedules, coverage, preparation, and implementation of all activities through a commitment to consensus building and teamwork. Rotating on-call coverage is required.

Qualifications & other important information:
• Personal stability, flexibility, and commitment to working with people recovering from chronic and persistent mental illness.
• Bachelor's degree in the Human Services field and/or four (4) years of experience working with people recovering from severe mental illness and substance use is preferred.
• Experience in substance use counseling is required.
• Be a Certified Psychiatric Rehabilitation Practitioner (CPRP) or be willing to achieve CPRP certification within one year of employment (training provided by the agency after a probationary period).
• Be CPR and First Aid certified or be willing to be certified (training provided by the agency).
• Must have a driver's license, a safe driving record, and reliable transportation.
• Excellent verbal and written communication skills.
• Proficient in the use of electronic medical records and the ability to input data, and to follow through to completion of services.
• Maintain confidentiality and adhere to HIPAA law, CARF accreditation standards, Maryland State Regulations, and other regulatory bodies.
• Assume responsibility for a caseload, generate member assessments, develop individual treatment plans, and document progress towards achieving goals; other related documentation requirements.
• Carry out assignments to fulfill the ACT mission of providing sanctuary and support to members in the community, including but not limited to: providing specialty expertise to the ACT team in substance use; regular mobile visits to people in the community; face-to-face and telephone contacts in accordance with the ACT Team's schedule. In coordination with other team specialists in areas including but not limited to: housing assignments to teach independent living skills in the areas of shelter, food, clothing, medications, money, and ADLs; assisting members to find and maintain housing; job placement and employment counseling; training in accessing community resources, transportation; positive role modeling; teaching basic standards of cleanliness and safety; member assessment and crisis interventions.
• Participate in agency employee meetings and daily ACT team meetings.
• Take part in a rotation of on-call for the ACT caseload evenings and weekends as scheduled.
• Ensure that all programmatic and physical environment issues are in accordance with agency policies and administrative regulations as appropriate.
• Assist in the coordination of services received from other community agencies.
• Other duties as assigned by the ACT Team Coordinator.

Job description:
Scope of Job: Assists the Health Home Nurse Care Manager to support service users in the development and maintenance of healthy lifestyles.

Job Duties:
1. Reviews updates at the beginning of work shift by: a) emptying and reading all correspondence from personal mailbox; b) reading and responding to email; c) listening and responding to voicemail; d) reading Anchor Update Report; e) covering assigned cell phone and responding to calls/texts.

2. Establishes a professional, caring relationship with all people being supported by the team. Has awareness of and is sensitive to each person's (and of co-worker’s) cultural, racial, and ethnic background. Develops and maintains relationships with hospitals and medical providers in the community to offer comprehensive services to service users.

3. Compares ongoing outcome data to baseline data to monitor each service user’s progress. Schedules contacts with service users. Assists to schedule medical appointments with external resources and follows up with service users. Assists in obtaining service users’ health records, labs, vitals, etc.

4. Communicates with Archway staff and all other disciplines on an ongoing basis to ensure coordination of care. Reviews results of laboratory tests and other diagnostic procedures. Identifies significant clinical findings, makes conclusions and intervenes appropriately. Supports and monitors compliance of the individual’s self-administration of prescribed medication, including checking of prescription labels and storage. Administers injectable medication as needed. Provides medication monitoring supports. Assists with the updating of medication logs; preparing medication charts, tracking, ordering refills and counting medication as needed. Oversees the quality assurance of medication charts. Interprets the appropriate information needed to identify each service user’s requirements relative to age-specific needs.

5. Stays up-to-date on somatic health issues and works with the nursing team to research and provide appropriate resources. Works as a team with other nursing staff to deliver quality services to focus on the improvement of the somatic health of people using the service, prioritizing prevention and education. Focuses on program goals specific to reducing emergency room visits and hospitalizations related to somatic health issues.

6. Monitors and maintains quality indicators and disease management indicators surrounding comorbid chronic conditions and healthcare screening, as indicated. Monitors and maintains appropriate reporting mechanisms, such as statistical data, as indicated.

7. Assists in providing leadership to the implementation and coordination of activities. Assists in training nursing staff. Assists in directing the activities necessary to obtain and maintain State of Maryland standards and CARF accreditation. Assists in maintaining compliance with all applicable licensures, certifications, and/or accreditation requirements and agency policies and procedures.

8. Provides training in areas including, but not limited to, communicable disease prevention as needed. Helps to promote services by attending community events and visiting other agencies and businesses in the community.

9. Focuses on meeting all COMAR regulations, CARF standards, and HIPAA laws. Attends certification/recertification training as scheduled, including maintaining licensure requirements as set forth by the Maryland Board of Nursing. Completes all other training as scheduled/assigned.

10. Performs other tasks related to the overall scope of the job as assigned.

The team provides services to people, meeting people where they are.

The ACT team works as a team to support people with obtaining housing, securing employment, and attaining educational goals. The team provides on and off-site services, including group activities and one-on-one contacts. You will not work remotely. Mobile work in the community is required.

Qualifications & other important information:
• Personal stability, flexibility, energy, and commitment to working with people with chronic mental illness.
• Formal training and/or related experience equivalent to a bachelor’s degree and/or 4 years of working with chronically mentally ill individuals and experience with helping people secure housing, benefits, education, recreation, workforce, and fiscal literacy.

• Demonstrates the ability to address the problematic needs of supported individuals and demonstrate familiarity with the psychosocial and fiscal literacy process.
• Work as a liaison to assist people in obtaining disability benefits, in collaboration with the SOAR Case Manager. If the person is not connected with the SOAR Case Manager, take the lead to assist people with obtaining these benefits.
• Excellent verbal and written communication skills.
• Assesses money management skills; provide tips for managing money.
• Participates in all ACT Team meetings and in-house trainings, and other meetings and trainings as requested.
• Keeps records, provide reports, and maintain data as required by the Mental Health Administration, or other regulatory agencies.
• Establishes program targets for income/fiscal literacy.
• Uses the electronic medical records, input data, and follow-through to see services to completion.

DUTIES
• Strategizes for the service users’ treatment, to provide housing, clinical stability, and
employment.
• Develops outreach and assessment ability to reach all parts of Allegany and Garrett
Counties.
• Provides direct therapeutic services as well as helps to determine best use of time to
provide team development, mental status assessment, intervention, and medication
reviews as well as supporting others in regular sessions with a caseload.
• Participates in a daily team meeting to review all members on caseload and develop
workable strategies for their treatment, to provide housing, clinical stability, and
employment.
• Uses the electronic medical record, inputs data, and follows through seeing services to
their completion.
• Maintains total confidentiality and adheres to HIPPA law, CARF accreditation standards,
Maryland State Regulations, ACT requirements, etc.
• Other duties as assigned by ACT Team Coordinator.

AUTHORITY: Position reports to ACT Team Coordinator and works directly with all intra-agency departments and other community providers.

JOB TYPE: Full-time

CLASSIFICATION, SALARY, AND BENEFITS: Regular, full-time position; exempt from
overtime. Grade 9a on salary schedule $64,900-$80,171 (Master’s Level).

There is a full-time (FT) position available with the Women's Recovery Program at Archway Station. This position includes full-time benefits following a successful 60-day probationary period. To be considered for the position, you should have a lived experience with recovery.

Full-time Women's Recovery Peer Specialist Position (30 hours per week - must be able to transition to working 40 hours per week as the program expands); Grade 2 on the wage continuum (wage range is $17.50 at step 1 - $21.18 at step 12), plus any credentialing incentives: You will work closely with the Care Coordinator to support the moms who live in the recovery houses with their recovery process and alleviate barriers that may interfere with their ability to obtain permanent housing and maintain sobriety, with focus on helping the moms with their independent living skills so that they can gain/regain custody of their children. You will encourage the moms to maintain a sober and healthy lifestyle, while assisting the Care Coordinator with supportive services and assisting them with self-sufficiency. You will provide support to appointments at DSS, the court system, housing, employment opportunities, mental and physical health appointments, etc. You will also assist the Care Coordinator with education regarding resources to help the moms maintain independence when leaving the program.

You will work the opposite end of the week of the Care Coordinator. The schedule is as follows (again, you will need to adjust your hours when the position transitions to 40 hours per week):

• Sunday ~ 9am - 3pm
• Monday ~ 9am - 3pm
• Tuesday ~ 9am - 3pm
• Wednesday ~ 9am - 3pm
• Thursday ~ 9am -3pm

Our Story

Archway Station's mission is to provide psychiatric rehabilitation services to adults recovering from serious mental illness and services to adults with developmental disabilities. Archway also serves children overcoming serious emotional and/or behavioral problems. The foundation of services is our belief that the children and adults using services are first and foremost people. The success of our service rests on the honoring and celebration of our shared humanity. The Early History of Archway Station (1979-1984)

In January of 1979 staff representatives of the Allegany County Mental Health Center and the Thomas B. Finan Center met to discuss Allegany County's need for a hospital-to-community transitional program for psychiatric patients. A task force was selected from representatives of the initial group. The primary responsibility of the task force was to develop a community-based psychosocial rehabilitation center. A consultant (Mr. Grady O'Rear) was hired by the State Regional Office to aid in the development of this program and a task force of representatives from mental health service providers was formed. The task force decided to follow the guidelines of the internationally-known Fountain House program of New York City. Therefore, the initial organization of Archway Station began in the second half of 1979.

Through the use of loaned staff, vehicles, and supplies from the Allegany County Mental Health Center and the Thomas B. Finan Center, a program was started. Initial members came to the program in the summer of 1979 from the Allegany County Health Department Day Treatment Program and the Thomas B. Finan Center. For several months this group of approximately eight persons met at the Mental Health Center and did recreational activities at the Finan Center.

In November of 1979, the group moved into the basement hall of St. John's Lutheran Church, 406 Arch Street, Cumberland, MD. The people using the service, originally referred to as “Clubhouse members”, chose the name “Archway Station”. The name chosen was likely influenced by the architecture of the church, as well as its Arch Street location – and remember Mr. O’Rear was from Way Station! Thus began the development of a private, non-profit organization. A Board of Directors was established and Articles of Incorporation were written and approved on April 4, 1980; Archway's official birthday.
