Vaginal Estrogen Safety for GSM: Genitourinary Syndrome of Menopause.

I wanted to talk you about some exciting news that happened this week for women dealing with Genitourinary Syndrome of Menopause, or GSM.

“GSM” is a common condition that affects women in perimenopause and menopause due to low estrogen levels. GSM can cause symptoms like va**nal dryness, persistent va**nal and vulvar itching, irritation, and even bladder leakage, burning when you urinate or p*e, va**nal dryness and discomfort with in*******se.

For years, many women have been hesitant about using estrogen cream because of an old black box warning suggesting safety concerns. But here’s the great news: this week, the FDA removed that warning—agreeing with years of scientific research that estrogen cream specifically does not increase the risk of heart attacks, stroke and breast cancer. This means estrogen cream is now recognized by the FDA as a safe, more reassuring option for relieving GSM symptoms. It can help restore your va**nal comfort, improve va**nal pH balance, and even provide hormonal support to your pelvic tissues to reduce the risk of GSM, bladder leakage and prolapse down the line. This is a real win for women.

I typically recommend my patients apply estrogen cream with the applicator that comes with the cream or a pea sized amount with your finger in the va**na and another pea sized amount outer va**nal opening, in between the minor l***a, an even some on the opening of the urethra or p*e hole. This combination addresses not only symptoms in the va**na but symptoms that irritate women at the opening of the va**na and urethra.

That’s one of the most common questions women ask me.

The answer is no.

A 50-year-old woman came in with severe prolapse of the uterus, bladder, and re**um. She had been managing symptoms like bladder leakage when sneezing, pressure in her va**na, and the feeling of something bulging down below.

She didn’t want to wear a pessary or keep adjusting something every few months. She wanted a real solution. A definitive fix.

We performed a va**nal hysterectomy using her native tissue. She also had bladder and rectocele repair, along with a collagen injection to help with stress urinary incontinence. All of this was done at a surgery center. After recovery, she was safely able to go home.

By preserving her ovaries, she maintained her hormone function. And because we used native support structures, there was no need for mesh.

This is a great option for women in their 40s, 50s, and 60s who have healthy tissue and want a natural, long-term result.
