Here is a recent case that shows why spinal fusion is often needed along with laminectomy (decompression) surgery. It also illustrates the importance of having a thorough evaluation done by your doctor. What makes a thorough evaluation is having a complete history and proper X-rays.

Image 1: This patient had spinal stenosis at L4-5 on MRI that looks straightforward as the first image shows.

Image 2: The X-rays revealed that he had severe spinal stenosis as the L4 and L5 as well as spondylolisthesis, which caused instability.

The third image is the postoperative X-ray taken six weeks after the L4 and L5 laminectomy and Transforaminal Lumbar Interbody Fusion (TLIF) back surgery.

In doing the TLIF spinal fusion, the surgeon stabilized the spinal vertebra. The result? It resolved all of the patient’s symptoms including his back pain and leg pain.

Here is an example of another cervical spine case: MRI and CT scans show a 54 year-old male who had a cervical fracture dislocation at C5-6 after diving forward into shallow water and striking his head. He was fortunate not to be paralyzed. The patient also had chronic conditions in his cervical spine that put him at even higher risk. He had severe cervical stenosis, or narrowing of the spinal canal. He also had a somewhat rare condition called OPLL or ossification of the posterior longitudinal ligament.

Surgery was be performed to take the pressure off the spinal cord and then realign his spine and stabilize it with both an anterior plate and screws and posterior screws and rods. The final x-ray as a postoperative picture. He did well after surgery and made a complete recovery with a return to normal activities.

Cervical refers to the neck while myelopathy is a disorder of the spinal cord. Thus, cervical myelopathy is a spinal cord compression in the neck. Myelopathy can cause the following symptoms: Neck pain or stiffness, tingling, weakness, or numbness in arms, fingers, hands, Balance issues that could affect walking, and trouble with fine motor skills such as buttoning buttons.

Here is a 64 y/o male who fell at home with resulting severe hand weakness and numbness due to cervical stenosis or narrowing of the spinal canal. The fall caused an injury to his spinal cord called myelopathy or a central cord syndrome. Central cord syndrome is a severe condition and can often result in permanent neurologic injury. Surgery (cervical laminectomy and fusion) was performed to relieve the pressure on the patient’s spinal cord and stabilize the spine. This patient responded very well to the surgery. In the days after surgery, he noted that his hands were stronger. By 6 weeks following surgery, he had recovered all of the normal function in his hands and arms.
