“A small act, a big impact”

During a recent Targeted Drug Delivery (TDD) Home Connect nurse visit, one of our patients wasn't feeling their best. A Home Connect nurse quickly noticed the signs and took thoughtful action, not only providing the scheduled pump care, but also tending to the patient’s immediate needs with compassion and attentiveness.

After completing the pump refill portion of the visit, the nurse offered the patient a couple of breakfast bars and a Gatorade to help replenish their energy. Within minutes, the patient perked up and began feeling better. Sometimes it is the smallest gestures that make the biggest difference.

This is just one example of the kind of whole person care our team brings to every visit. It is not just about medical treatment, but about seeing the full person, responding to the moment, and showing up with empathy.
