Prominent ears (ears that stick out too far) can be a source of teasing and can negative impact confidence. Parents frequently ask when is “the best time” to do an otoplasty to correct the shape and projection of the ears. I frequently say the day before your child is ever teased or feels self conscious about their ears. We obviously can’t predict that. The earliest I offer otoplasty is around age 5. Otoplasty can be performed in anyone after that age, so the “right time” is very much dependent on the individual child. Parents know their kids best and will make the best decision on the timing for otoplasty. This is a very rewarding procedure to perform as the immediate boost in confidence for the patients is easy to see at all ages. This patient is shown before and just 6 weeks after her otoplasty.

People with unilateral cleft lip have an asymmetrical and crooked nose. An uneven appearance of the nostrils and nasal tip are stigmatizing and draw unwanted attention that can impact self esteem. This beautiful patient wanted improved nasal symmetry and a more delicate appearance to her tip. She is shown before and 1 year after cleft rhinoplasty. strong

I am excited to announce that we’ve added the newest ultrasonic (piezoelectric) saw on the market, the Piezomed Pro from W&H, to our rhinoplasty armamentarium. Ultrasonic rhinoplasty (also called piezoelectric rhinoplasty) uses ultrasonic vibrations to cut and shape the nasal bones instead of traditional chisels and hammers. Its main advantages include:

Twenty years of research, innovation, and clinical experience continue to refine my approach to cleft care.

This publication of mine from 2024 in The Cleft Palate–Craniofacial Journal explores alar asymmetry in unilateral cleft lip nasal deformity, analyzing shape, volume, and axis rotation in skeletally mature patients.

The ala is the fleshy part of the nostril. In many cases the ala is significantly smaller on the side of the cleft lip and that contributes significantly to an uneven appearance of the nostrils. This asymmetry is often overlooked by surgeons and left untreated. I hope this paper will help improve outcomes for all patients undergoing cleft rhinoplasty. 🌟
