🏥 We’re proud to announce that Foremost Family Health Centers has earned NCQA Patient-Centered Medical Home (PCMH) Recognition for 2026!
This milestone reflects our ongoing commitment to delivering high-quality, patient-centered care to our community, especially in neighborhoods that have historically been underserved. As a federally qualified health center (FQHC), we know that achieving this recognition is not just a badge, it’s a reflection of how we deliver care.
At Foremost, we leaned into each of these areas, tailoring our workflows, data systems, and care strategies to meet the needs of our patient population.
As an FQHC, Foremost serves a diverse community including low-income, uninsured, underinsured, and Medicaid populations.
Our clinics (e.g. Martin Luther King Jr. Family Clinic, Balch Springs, school-based sites) handle patient volumes across age groups (infants, children, adults, seniors) and multiple service lines (medical, dental, behavioral health, etc.).
Many of our patients live at or below 200 % of the federal poverty level (a common threshold in FQHC settings). (Note: in FQHCs nationally, > 90 % of patients are low-income, many under 200 % FPL)
Key Focus Areas in Achieving Recognition
Here are some of the strategic levers and improvements we prioritized:
1. Leadership & Practice Organization / Team-Based Care
• Governance alignment with patient voices and community representation
2. Knowing & Managing Patients
• Enhanced data collection (social determinants, risk stratification, registry management)
3. Access & Continuity
• Extended hours, same-day or walk-in access, after-hours coverage
4. Care Management & Support
• Identifying higher-risk patients (e.g. multiple comorbidities, no-shows, social complexity)
5. Care Coordination & Transitions
• Collaborations with specialty care, hospitals, social services
6. Performance Measurement & QI
• Continuous quality improvement cycles and root-cause analyses
Why this matters for our patients and our community, this recognition means:
- Better preventive care and chronic disease outcomes
- Improved access, fewer delays, and more consistency
- Smarter use of our limited resources
- Stronger credibility and partnership opportunities with payers, health systems, and public health agencies
