From just one easy appointment, health plan members receive a detailed picture of their health, with the time to get their questions answered. After the visit, Signify Health shares a comprehensive report with the member, their primary care provider and their health plan. Our proactive approach enables members, health plans and health care providers to act on the information we provide efficiently and effectively. No matter the season, Signify Health clinicians can come to you for a $0 cost In-Home Health Evaluation. The visit will last up to an hour and the clinician will:

▪️ Go over your medicines with you
▪️ Check your health, like your blood pressure
▪️ Test your memory, if needed
▪️ Look for other things that could put you at risk, like falling
▪️ Answer health-related questions
