WHO CAN ATTEND?
• School-aged athletes with a NEW sports-related injury (excluding head injuries).
• Note: Concussions cannot be evaluated.
• We can only treat one injury per visit.

WHAT TO EXPECT?
• X-rays and initial exam by our provider are included.
• Bring insurance cards and/or school claim forms for any additional studies (e.g., MRI).
• If follow-up care is needed, an appointment will be scheduled during regular clinic hours (billed accordingly).

PARENT/GUARDIAN CONSENT:
• A parent or guardian must attend if the athlete is a minor.
• Coaches or Athletic Trainers may bring a signed authorization to treat a minor form.

REHAB & REFERRALS:
• Rehab may be provided by the school Athletic Trainer when applicable.
• Local physical therapy (PT) may be prescribed if needed (billed accordingly).
• Some athletes may require a primary care referral for further treatment.

South Arlington Wednesday Injury Clinics
WHEN: Wednesday Evenings 5:00 – 6:00 pm
WHERE: 4401 Park Springs Blvd, Suite 130 *Must Check-In by 6:00 pm Arlington, Texas 76017
This Season: February 5 – April 30, 2025

WHO: School-aged athletes with a NEW sports-related injury that have not had previous treatment by a doctor for their injury (excluding head injuries).
Please note: CONCUSSIONS CANNOT BE EVALUATED. WE CAN ONLY TREAT ONE INJURY PER VISIT.

VISIT: X-rays and initial exam by provider are included. Please bring insurance cards and/or school claim forms to present at check-in in case further studies (i.e. MRI) are needed which will be billed to patient’s insurance plan. Patients with injuries needing follow-up care will have appointment scheduled during regular clinic hours, then billed accordingly.

CONSENT: Parent or guardian attendance is required when the athlete is a minor, or athlete’s Coach or Athletic Trainer MUST bring an authorization to treat a minor form signed by the parent or guardian.

REHAB: Rehabilitation may be provided by the school Athletic Trainer, when available or applicable or local physical therapy may be prescribed. If physical therapy (PT) is prescribed and patient schedules PT visit, patient will be billed accordingly.

REFERRAL: Some athletes will be required to see their primary care physician to obtain referral for further care when appropriate.
