WHO CAN ATTEND?
β€’ School-aged athletes with a NEW sports-related injury (excluding head injuries).
β€’ Note: Concussions cannot be evaluated.
β€’ We can only treat one injury per visit.

βΈ»

WHAT TO EXPECT?
β€’ X-rays and initial exam by our provider are included.
β€’ Bring insurance cards and/or school claim forms for any additional studies (e.g., MRI).
β€’ If follow-up care is needed, an appointment will be scheduled during regular clinic hours (billed accordingly).

βΈ»

PARENT/GUARDIAN CONSENT:
β€’ A parent or guardian must attend if the athlete is a minor.
β€’ Coaches or Athletic Trainers may bring a signed authorization to treat a minor form.

βΈ»

REHAB & REFERRALS:
β€’ Rehab may be provided by the school Athletic Trainer when applicable.
β€’ Local physical therapy (PT) may be prescribed if needed (billed accordingly).
β€’ Some athletes may require a primary care referral for further treatment.
