2018: I find one area of discussions quite helpful in my various approaches in clinical pastoral care [palliative] is advance care planning. Implications for service planning and development [https://www.ncbi.nlm.nih.gov/pmc/articles/PMC557152/] that "ONE SIZE MAY NOT FIT ALL" Different models of care will be appropriate for people with different illness trajectories...the model of cancer palliative care - for example - might not suit people who have a gradual, progressive decline with unpredictable exacerbations. What I am discovering in this area of pastoral and narrative counseling is that families in crisis need an "active listening ear" before giving input, feedback and valuable resourceful information prior to executing such documents [Living Wills, Advance Directives to Physicians, Medical Powers of Attorney, Out of Hospital DNRs, etc]. But, on a good note most people may not understand, or have misconstrued is that modifications can be updated if the person [patient, client or consumer] is coherent times three and willing to make such modifications to their treatment options, end of life care wishes, change in agents, surrogates and those list as proxy; and most importantly, the cost is minimum to none. In addition, mind you; such discussions and conversations may draw families closer in harmony respecting the choices of loved ones or may cause others [believe it or not] to isolate or withdraw themselves to engaging in further care and connection to the the patient's treatment team. Alongside one's church clergy, ministers, rector, priest, pastor, spiritual care advisor or counselor, please be mindful how cutting oneself out of the equation of care for your loved one may impact your individual coping mechanisms should you face an unfortunate crisis in medical care. When a person is admitted to a hospital environment; be it, an emergency trauma coded as level one, two or three [one being the most severe cases]; one of the questions families do not hear is the question, "do you have an advance directive?" This question is sometimes asked again when getting day surgery, readmitted to the hospital or transferred within the hospital environment prior to an invasive and more aggressive treatment is considered. Check out some of the readings on the work of Joanne Lynn and colleagues at the Washington Home Centre for Palliative Care Studies, and June Lunney and colleagues at the National Institute of Aging, National Institutes of Health, Bethesda, USA on such discussions on not just end of life care but varied illness trajectories and how you, my friend, can help in the decision process respecting the goals, values and wishes of your loved ones.
Please consider discussion [coded as Consultation] on Advance Care Planning with your primary care physician, your local medical social worker [an LMSW in most hospitals], a certified Facilitator of Advance Care Planning or your local hospital or hospice care chaplain or should your loved one currently be aging in place within a long-term care nursing home and skilled facility; ask for the facility ombudsman or social worker. Simply ask for information. Most locations have pamphlets and pertinent documents you simply can review on your own prior to scheduling an appointment to have conversations of such an otherwise, sensitive but meaningful subject.
