BTW, please make sure we have a copy of your insurance card on file (scanned, etc). Especially if your insurance changed for the year (or even a new secondary insurance). You can either let the front desk scan your card, or you can email us w/ a picture/scan of your card at mountainviewchirovt@gmail.com.
