✏️ Back-to-School Reminder! 📚🍎

Parents, please be sure to check with your child’s school to see if a special physical form is required for the upcoming school year.

👩‍⚕️ Many schools happily accept the standard form we provide in the office after your child’s wellness exam. However, some schools may require a specific school-issued form. Also, some schools require particular sports forms. Always check with your school first.

⏳ If we need to complete a form that isn’t already on file, it may take up to one week to process. (Good news—we try to keep a copy of any forms completed at the time of the wellness exam, so they’re ready if you need them!)
