Dear Patients,

I have enjoyed caring for you and your families over the past ten years. With a mix of joy and sadness, I am writing to let you know of my intention to close my Davenport, Iowa office. Our last day open will be on July 31, 2022. Our Burlington, Iowa office will remain open.

Please be assured that my staff and I will do everything we can to make the transition smooth and stress free. I recommend that you begin looking for a new optometrist as soon as possible.

Your medical records are confidential and will remain on file with EyecareMax. You may pick up your records and current eyeglass prescription during regular business hours before August 1, 2022. We cannot share your medical information without your consent. Please send a signed note if you have someone pick up your records.

It has been a great pleasure meeting and caring for you all. I sincerely appreciate your friendship and loyalty. I wish you continued good health and all the best in the coming years.
