Bupropion — If a patient with a distressing sexual problem greatly desires a pharmacologic intervention, after nonpharmacologic treatments have been tried, bupropion is often the first choice. Bupropion is FDA approved to treat depression and assist with smoking cessation; use for SSRI-induced sexual dysfunction is an off-label indication. As a centrally acting agent, the mechanism of action is likely similar to that of flibanserin. Long-term safety data are available, and risks and side effects are well characterized. Generic formulations are available, so cost is low. Bupropion should be dosed in the morning, and patients should be observed for increased anxiety, insomnia, and hypertension.

The morbidity and mortality associated with being overweight (body mass index [BMI] of 25 to 29.9 kg/m2) or having obesity (BMI of ≥30 kg/m2) have been known to the medical profession for more than 2000 years . Obesity is a chronic disease that is increasing in prevalence in adults, adolescents, and children, and is now considered to be a global epidemic. Screening for obesity can identify high-risk patients who may not otherwise receive counseling about health risks, lifestyle changes, obesity treatment options, and risk factor reduction. Evaluation of a patient who is overweight or with obesity should include both clinical and laboratory studies; the combined information is used to characterize the type and severity of obesity, determine health risk, and provide a basis for selecting therapy.

The most obvious source of financial impact for patients suffering from obesity compared to healthy-weight individuals is direct medical costs. In a recent study, that number has increased by about 30% to $1900 per year due to rising US health care costs and the need for more health care services, especially for those related to chronic disease care. Research has shown that over the course of a lifetime, increased individual direct costs range from $9,000 to $17,000 compared to healthy-weight counterparts.

According to the CDC, six in ten US adults have at least one chronic disease, and four in ten suffer from two or more. As we know, there is a direct relationship between obesity and the risk of developing over 50 chronic conditions. In short, excess weight also adds extra individual financial burden.

Women inquiring about planned home birth should be informed of its risks and benefits based on recent evidence. Specifically, they should be informed that although planned home birth is associated with fewer maternal interventions than planned hospital birth, it also is associated with a more than twofold increased risk of perinatal death (1–2 in 1,000) and a threefold increased risk of neonatal seizures or serious neurologic dysfunction (0.4–0.6 in 1,000).
