ROBOT 🤖 DAY- the ‘DaVinci’ Surgical system has been a widely utilized technology amongst multiple surgical disciplines. {pic #2} It could provide surgeons with better self-sufficiency as well as enhanced visualization & precision. For patients it offers shorter operative time or time under anesthesia, quicker recovery and often same day discharge! Case above involves female in her 40’s suffering from abnormally heavy periods & chronic pelvic pain due to multiple large fibroids for several years. Robotic-assisted laparoscopic hysterectomy (removal of uterus, cervix and the fibroid tumors {pic #4}) was performed with modest blood loss and the pt was discharged home few hours after surgery. 🤖🤖🤖

ENDOMETRIAL ABLATION- is a minor procedure used to treat abnormal uterine bleeding (AUB) or heavy bleeding with regular or irregular periods. It is an option for patients who failed medication 💊 management to control bleeding and a good alternative to more invasive surgical treatment (hysterectomy). Procedure includes insertion of thin device (pic #1) trough cervical canal into the uterus interior & utilizes an energy source to destroy the inner lining of the uterus (endometrium); most commonly a radio-frequency energy that takes 1-2 mins on average to complete ablation procedure (pic #2) . No incisions or post-operative down time required with this kind of procedure. Case above includes 39 year old patient with recent history of stroke and having to be on anticoagulation therapy (blood thinner) which resulted in heavier and longer periods resulting in anemia. Given need to be on blood thinner; invasive surgery was not going to be the best option for this patient therefore endometrial ablation was performed; pic #3 ‘before’ & pic #4 ‘after ablation’. Patient doing great post-operatively with no pain and over 90% reduction in her menstrual bleeding 🩸

Follicular Cyst- or the common ‘ovarian cyst’ originates from a normal physiologic process of follicle formation that occurs monthly in women with regular cycles. Occasionally some of those follicles persist after ovulation has occurred and fill up with fluid or blood and could expand to abnormally large cysts. Most common symptom is pelvic pain that tends to be on the side of the existing cyst. Case above is of a young woman in her 20’s with chronic pelvic pain on the right side. On CT imaging a enlarged mass was noted coming off her right o***y; 13.5 cm in diameter and pushing up against her ureter on the right side causing swelling in the outflow tract of the kidney on that side (Hydronephrosis). Diagnostic laparoscopy was performed revealing ovarian cyst on right side (pic #2). The cyst was punctured and fluid completely suctioned leaving redundant wall of now deflated cyst (pic #3) that was then resected and sent to pathology (pic #4). Patient did well post-operatively and went home the same day!

FETAL MACROSOMIA- describes a newborn who's significantly larger than average. A baby who is diagnosed as having macrosomia weighs more 4,000 grams (8lb13oz) regardless of their gestational age. Approximately 8-9% of babies worldwide exceed this weight at birth.
Risks associated with fetal macrosomia increase greatly when birth weight exceeds 4,500 grams (9lb15oz).
Fetal macrosomia may complicate vaginal delivery and put the baby at risk of injury during birth (shoulder dystocia or trapped fetal shoulder is a commonly feared complication). Fetal macrosomia also puts the baby at increased risk of health problems after birth. Some signs of possible underlying macrosomia at prenatal visits include a large fundal height measurement {pic2} or excessive amniotic fluid (polyhydramnios) on ultrasound. Factors increasing risk for fetal macrosomia include; maternal diabetes, obesity and history of having macrosomic births.

Uterine Polyps- formally known as ‘endometrial polyps’ are the result of overgrowth of cells in the uterus’s internal lining that could bundle growing off a base or a stalk and appear as finger-like projections into uterine cavity. Cause of polyps isn’t fully known but thought to be related to higher than normal estrogen exposure of the uterus lining. Most commonly found in women around the menopause age range but can also occur in younger women. Majority of endometrial polyps are benign but some could be cancerous or carrying pre-cancerous cells. Most common symptoms of a polyp is heavier period bleeding or bleeding in between periods or at random times for a varying length of time. If a polyp is large enough it could interfere with fertility and make conceiving process more difficult. Case above includes 48 year old female with recent onset of heavier than normal periods and having episodes of uterine bleeding in between periods. Patient taken for surgery; hysteroscopy with polyp removal performed. Symptoms resolved & patient doing well post-operatively.

Ovarian Cystadenofibroma- is a relatively rare type of tumors of the o***y that contains a mix of both epithelial & fibrous stromal cells (giving its mixed soft & hard components grossly-pic #4). It is not a malignant (cancerous) tumor; however on diagnostic imaging can usually seen as a ‘complex’ mass containing cystic & solid components and could resemble a malignant tumor. This case includes a 70 year old female with chronic pelvic pain specifically in her right lower quadrant for about 4-6 weeks; symptoms present daily with pain not always responsive to Advil. Ultrasound revealed 4x6x3.5 cm complex mass on her right o***y. Pt taken for surgery: diagnostic laparoscopy & enlarged right o***y with mass noted (pic #2&3). Right o***y & tube were removed and sent to pathology confirming diagnosis. Patient doing well post-operatively and pain significantly improved.

Neural Tube Defects (NTD) occurs in about 3,000 births every year in the US. Underlying causes include genetic and nutritional deficiency in folic acid. The neural tube is an embryonic structure that closes at 4 weeks of gestation to form brain and spinal cord. Most common types of NTD’s are spina bifida (lower defect as seen in pic #1) & anencephaly (higher defect affecting brain 🧠 formation). Folic acid supplementation present in prenatal vitamins & diet play a significant role in prevention and lowering incidents of NTD’s. Common recommended doses of folic acid are 400 micrograms per day in all pregnant women and increased up to 4000 micrograms in women with a history of birth affected by NTD; to be consumed 1 month prior to conceiving and for the first 3 months of the pregnancy.

Ovarian Vein Thrombosis- rare condition typically seen post-delivery or post-operatively usually with underlying hyper-clotting disorder in the patient. This patient presented to ER with severe abdominal pain and nausea that started suddenly; was thought to have an ovarian torsion when no blood flow to her right o***y was noted on ultrasound imaging. Pt had a clotting disorder known as Antithrombin III deficiency that significantly increases risk of forming blood clots in the deep veins (Deep vein thrombosis). Patient was taken to operating room; where vascular surgeon placed an IVC filter (pic #2-device to mechanically traps deep vein clots from traveling up to the heart and lungs that could lead to death) followed by diagnostic laparoscopy and removal of right o***y that appeared necrotic (pics #3&4-dead tissue from lack of blood flow). The o***y was NOT twisted around its blood supply (torsion) but rather the blood vessels supplying o***y with oxygenated blood were occluded from clots that had formed inside!
