Our services are available to you both long and short term assistance, regardless of your age, disAbilities or illness. We provide S.O.U.R.C.E. Service Options Using Resources in a Community Environment (SOURCE) links primary medical are and case management with approved long-term health services in a person's home or community. SOURCE operates under authority of the Georgia Elderly and Disabled 1915c Medicaid Waiver and provides Home-and-Community-Based Services to frail elderly and physically disabled people who meet the Intermediate Nursing Home Level of Care.
