Delta Projects is looking for a Facilities Specialist to join our team! Reporting to Director of Facilities, the Facilities Specialist will be responsible for general repairs, maintenance, and renovation of Delta Project’s properties. The position will work closely with the Director of Facilities in day-to-day scheduling and work order management. The ideal candidate has a background in facilities and/or trade such as carpentry, electrical and plumbing.

ESSENTIAL FUNCTIONS:
Complete maintenance, repair tasks and cosmetic improvements at organizational properties as instructed by Facilities Director.
Performing minor plumbing and electrical systems repairs.

Supervise work crew, contractors and other service providers engaged at Delta Projects properties.

Available to respond to emergency calls as required.

Process and track requests for repairs and maintenance from Residential Managers using online work order system (TheWorxHub).
Assist in preparing for regulatory inspections, funding source audits and other property reviews by insuring homes are safe and well maintained.

QUALIFICATIONS:
2 – 5 years’ experience in related facilities maintenance and repairs position and/or carpentry, plumbing, electrical or HVAC field.
Ability to foresee problems, implement solutions and complete ordinary repairs in a timely manner

Working knowledge of building systems, vendor relations, and industry inspections.
Demonstrated knowledge of building trades including plumbing, electrical, HVAC and carpentry

Knowledge of applicable codes and regulations related to facilities, residential facilities and building system operations
Proven ability to work independently
High School Diploma or equivalency
Qualifying criminal background and driving record

Must possess a valid driver’s license in state of residence.

Are you an LPN and looking for a rewarding experience working with individuals with disabilities who live in a group home setting? Delta Projects is looking for a part-time LPN Case Manager who will be responsible for rendering nursing care for individuals with Acquired Brain Injuries in a group home setting located in North Attleboro, MA. Expected schedule includes 20 hours on site in North Attleboro.

$2000 Sign-on Bonus included! (pay out schedule determined by Employer).

As an ideal candidate, you have already worked as a human services professional in a group home contracted by DDS. You’re able to work independently and utilize established procedures, policies, guidelines, and standards to make the Nursing department more effective. You will be responsible for assessing clients, review physical exams and ordered treatments, and properly follow up and coordinate medical and psychiatric care. As an ideal candidate, you’ll be able to carry out physician orders accurately, directly, and indirectly as well as coordinating communication around physician orders, patients’ changing needs and problems to patient, staff, Registered Nurse and Physician, as needed. You will assist in data collection on all medical data records. You’ll gain experience reviewing medication compliance and training staff, patients, and guardians.

The LPN Case Manager will be responsible for communicate with Residential Manager, DRS, and DON on site around proper planning, training, and appointments that require nursing input. In this position, you will be responsible for conducting weekly assessments performed with individuals – such as full vitals, skin integrity assessments, ROM, Med review etc. You will document and report end of day narrative notes by emailing the nursing team and discuss and address any issues obtained from narrative notes with the RM to determine any need for training or retraining. You will conduct on-going trainings with staff and any new staff on protocols and supportive and protective devices and review, revise and or develop any protocols needed for the individuals.
You will attend pertinent medical appointments, liaison with medical providers, report all findings to the residential manager and the team, and discuss medical updates with the Residential manager at least weekly to review and update on current site status/concerns and plans of care.

QUALIFICATIONS:
Graduate of an accredited school of Nursing

Current and valid LPN license in the state of Massachusetts.

Current AHA Healthcare Provider training (BLS)

Current and valid driver’s license in the state of residence

ABI experience a plus

Qualifying criminal background and driving record

Professional oral and written communication skills

Proficiency with Microsoft Office

Ability to consistently evaluate own work and effectively be able to collaborate and problem solve with various teams

Delta Projects is hiring Senior Instructors to work in our residential homes supporting individuals with intellectual and developmental disabilities to live and grow in a manner consistent with that of other citizens of the Commonwealth. Delta promotes dignity and respect for each individual while also focusing on individual needs, hopes and desires.

Senior Instructors will:
Be responsible for maintaining a safe, supportive and therapeutic environment which fosters growth and development while facilitating mutual aid and self-help skills among individuals.

Work together with the people we serve, their families, friends, employers, and other support staff to transform their lives by either building independence skills or increasing each person’s quality of life.

Provide direct support to people with intellectual and developmental disabilities in a small group residential setting or day program.
Administer medication under the Medication Administration Plan (MAP), implement treatment plans, and assist people with activities of daily living, such as household chores, cooking, and person hygiene.
Strengthen residents’ connection to their community by providing transportation on outings such as shopping and doctor’s appointments.

Perform all duties and responsibilities of an Instructor/ Direct Support Professional
Assist the Residential Manager in conducting staff interviews, orientation and training sessions.

Assist the Residential Manager in ensuring upkeep of property.
Perform on call responsibilities in Residential Manager’s absence.

Requirements:
Minimum of a High School Diploma or equivalent;

Direct care experience working with adults diagnosed with Intellectual Disabilities preferred.

Must be able to demonstrate competence in certain areas of training including but not limited to CPR, First Aid, non-violent physical interventions, behavior management, medication administration, and driving

Requirements:

High school diploma or equivalent. MAP certified preferred.

All applicants must be willing to undergo Medication Administration training, if they are not already certified.

All applicants must be willing to go through a two-week paid new hire orientation.

All applicants must have a current, valid, US drivers’ license for at least one year.

All applicants must have a reliable vehicle, car insurance, and clean driving record.

Delta Projects is looking for a Residential Manager for an Acquired Brain Injury home in North Attleboro, MA.

The Residential Manager is responsible for establishing and maintaining a safe, supportive, and therapeutic environment, all while facilitating mutual aid and self-help skills among the individuals we serve.

The ideal candidate is able to ensure proper supports are secured. He/she must be able to recognize when an individual is in crisis mode and can properly de-escalate the situation. The ideal candidate will be able to foster and supervise a team of direct care staff, which includes training, managing staff schedules and coverage, performance evaluations, and other supervision requirements. He/she will be able to assist individuals in gaining services and benefits that they are qualified for.
Lastly, the right candidate will be able to simultaneously complete these tasks all while being responsible for administrative duties such as auditing individual records and completing business reports (payroll, census, petty cash), completing programmatic documentation and assessments (ISP assessments, behavioral data reports, etc.). We are looking for a candidate who will be an excellent advocate and role model for the individuals we serve, among other duties assigned.

Requirements:

* Minimum of a High School Diploma or equivalent AND at least one year of experience as a supervisor or manager in a residential setting for adults –contracted with the Department of Developmental Services. Bachelor’s Degree preferred.
* ABI experience a plus
* Leadership and/or management experience required
* All applicants must have a current, valid, US drivers’ license for at least one year.
