Weight today 271 lbs (unchanged from 2/17). • Urology: Dr. Leaf ordered repeat labs (estradiol, free & bioavailable testosterone, prolactin, FSH, LH) and will discuss treatment options (shots vs oral testosterone) at a visit scheduled in ~3 weeks. Prior testosterone on 2/17 was 446 ng/dL; Dr. Leaf considered this normal for patient’s age but plans to re-evaluate. • Prior CBC (10/11): Hgb 16, Hct 47.5, Plt 208 — sent to Dr. Leaf. NP notes CBC should be updated before any testosterone therapy. • Sleep: Patient snores and reports significant daytime exhaustion but has not completed a sleep study; prior attempt to schedule yielded no callback. Provided contact for Dr. Lau (Bagchandani), pulmonologist/sleep specialist (954-917-4941). Instructed patient to call insurance (BCBS FL) for additional sleep medicine options and to notify office once scheduled. • Fatigue likely multifactorial (possible sleep apnea and hormonal factors). • Diabetes: Last A1C 6.2; eyes clear—no retinopathy per recent exam. • Hyperlipidemia: Last LDL 101 (10/10); on ezetimibe (“S-Stimivi”) every other day due to statin intolerance. • Current medications include Farxiga, hydrochlorothiazide, and ezetimibe qod. • Lifestyle: Attempting to stop eating after 7 pm; exercise limited by fatigue.

COMMON STI MISCONCEPTIONS YOU SHOULD KNOW

Many people think a “full STI panel” means everything — it doesn’t.
HSV-1 and HSV-2 (herpes) are not routinely included unless you specifically ask.

You also don’t need symptoms to have an STI.
Chlamydia, gonorrhea, HPV, herpes, and even HIV can be silent for months or years.

Condoms reduce risk — but skin-to-skin infections like HPV and herpes can still spread.

Annual exams do not automatically include STI testing.
Always ask what you’re being tested for.

Knowledge = protection.
Ask questions. Get tested. Protect your health.

Many African Americans naturally have a lower white blood cell (WBC) count, a normal variant known as Benign Ethnic Leukopenia (BEL). This genetically mediated finding is not associated with immune deficiency, increased infection risk, or underlying disease. Individuals with BEL typically remain completely asymptomatic, and the lower WBC/ANC represents their normal physiologic baseline.

Clinical concern arises only when low counts are accompanied by symptoms such as fever, recurrent infections, weight loss, or additional abnormal laboratory findings.
