CHP Defiance Inpatient Hospice Center CHP Home Care & Hospice of Defiance
24-hour hospice care center addressing the needs of terminally-ill patients and their families

There are times when a terminal illness cannot be adequately managed at home. CHP offers inpatient hospice centers in Defiance and Van Wert that specialize in pain and symptom management so that patients can return to wherever they call home. What makes CHP Inpatient Hospice Centers different is that all hands-on care is provided by specially trained hospice nurses. We give 24-hour care that minis

ters to physical, emotional and spiritual concerns. The unique design of our 12-bed centers addresses the needs of the whole family. All rooms are private suites with immediate access to skilled nursing care and an on-site social worker. Families have access to a chapel, kitchen, library, plus showers and overnight accommodations. There is space for family to gather and quiet places to be reflective. Care is available on a short-term or long-term basis.

We are deeply grateful to an anonymous donor for their generous gift of 25 folding chairs to our inpatient hospice center.

These chairs will make a meaningful difference in the care environment for our patients, families, and staff - offering comfort during some of life’s most tender and important moments. Every seat represents added support, dignity, and presence for those we serve.

While we may not know who to thank personally, please know your kindness is seen, felt, and deeply appreciated by our entire hospice team and the families we walk alongside each day.

From all of us—thank you for making a lasting impact through your generosity.

🩵Looking for a meaningful way to give back?

CHP Home Care and Hospice is offering Hospice Volunteer Training—we are looking for compassionate individuals to join our volunteer team.

Sometimes the most powerful gift you can give is simply your presence. Whether it’s sitting with a patient, offering a listening ear, or supporting families, your time truly matters.

If you have a heart for helping others, we would love to hear from you.

Volunteers don't look for the limelight, but they certainly deserve it. 💙
This National Volunteer Week, we are celebrating the wonderful hearts who serve at the CHP Defiance Inpatient Hospice Center. Whether they are the warm smile greeting families at our reception desk or the gentle presence visiting at a patient’s bedside, their impact is immeasurable.
We loved getting to celebrate them at our appreciation dinner this week. Thank you for choosing to spend your time bringing comfort to our patients and families!
