STAR PLUS NON-WAIVER/PHC
Requirements are eligible for Medicaid, diagnosed with a medical disability or condition & 21 or older.
STAR PLUS WAIVER
Requirements are: (1) must have disabilities or a medical necessity criteria for nursing home care; and (2) must be 21 or older.
COMMUNITY ATTENDANT SERVICES (CAS)
Requirements are diagnosed with a medical disability or condition, income specified limited & any age.
FAMILY CARE (FC)
Requirements are must have an unmet need or home management care tasks, income specified limits & 21 or older.

• Transfer & Ambulation

We service the following counties: Atascosa; Bandera; Bexar; Dimmit; Edwards; Frio; Kendall; Kerr; Kinney; La Salle; Maverick; Medina; Real; Uvalde; Val Verde; Webb; and Zavala.
