79 year old gentleman presented with Right sciatica for 6 months. Despite trying medications and spinal injections his ability to walk was markedly limited. His MRI demonstrates spinal stenosis particularly at the L4 L5 disk level and slippage of the vertebral bones (spondylolisthesis). He underwent a TLIF a procedure to take the pressure off of the nerves and correct the slipped vertebra. A “cage” is placed in the disk space to allow fusion. He was discharged the day after the procedure.

Interesting article which shows that cortisone shots, such as epidural and facet joint injections, do not increase a patient’s risk of fracture even in patients with osteoporosis.

We are often asked by patients if it is safe to have spinal injections if they have osteoporosis. This study may help to answer that question.

Here is a recent case involving a patient who experienced a year of significant neck pain, accompanied by radiating discomfort in both arms and hands. An MRI revealed substantial collapse in the disc spaces, leading to compression of the nerve exit points in the spine and subsequent radiation of pain to the extremities.

The patient underwent surgery utilizing the standard anterior approach, during which the degenerated discs were excised and replaced with spacers to restore disc height and alleviate nerve compression. A thin plate was then positioned and secured over the affected disc spaces to facilitate immobilization and promote healing. Following the procedure, the patient experienced complete relief from arm pain, which had persisted for a year prior to surgery.

35 year old gentleman with unremitting sciatica. MRI demonstrates sizable herniated disk at lowest disk in lumbar spine (L5 S1). We did a microdiskectomy through a one inch incision using microsurgical techniques. Patient was up and walking and was discharged to home 2 hours after procedure.

Recent case of 66 year old gentleman who had a Laminectomy a few years ago. Presented to us with severe back and sciatica pain. He had tried numerous injections into his back and various medications. He underwent corrective surgery to take the pressure off of his pinched nerves and to straighten out his scoliosis. Walking upright with no more sciatica.

Lady in her 80s who presented to us with progressive loss of balance and arm weakness and numbness. Typical symptoms of cervical stenosis and cervical spinal cord dysfunction know as myelopathy. MRI demonstrates significant spinal cord compression at the lowest area of the neck with injury to the spinal cord. She underwent a one hour procedure to unpinch the compressed nerve and was discharged the following day.

Today’s case… Gentleman with intractable sciatica and spinal stenosis at two levels (L4-5 and L3-4). Using microsurgical techniques we were able to take the pressure off of the nerves through a 2 inch incision and no need for fusion by preserving as much of his normal tissues as possible.

Case of the day… Gentleman with severe spinal stenosis and unrelenting sciatica pain underwent procedure to take the pressure of the pinched nerve in his lower back and to stabilize the area. Medtronic Stealth computer navigation and intraoperative Cat Scan is used to ensure safe and proper placement of the hardware.
