New CMS HCBS Quality Measures: Early Signals for Future Contract and Compliance Expectations
CMS has released a proposed 2028 Medicaid Home and Community‑Based Services (HCBS) Quality Measure Set that would require states to report on 23 core quality indicators, with several additional measures offered voluntarily. Although the reporting responsibility rests with states, not individual providers -the measures emphasize person‑centered care, safety, dignity, meaningful community engagement, and goal attainment, all of which directly reflect provider practices.
While CMS does not anticipate major new data‑collection burdens, the real impact will depend on how each state operationalizes the measures. Over time, states or managed care plans may embed these quality concepts into contracts, oversight activities, or value‑based purchasing strategies, making it essential for HCBS providers to ensure their operations, workforce training, and care planning processes are aligned with these priorities.
Public comments are due within 30 days of Federal Register publication, offering providers and stakeholders an important opportunity to influence implementation.
