Located on the Eastern Shore of Maryland, Choptank Community Health System, Inc. (CCHS) is a private, non-profit community health center providing primary health care services in Caroline, Dorchester, and Talbot Counties and the surrounding areas. CCHS health centers provide a full range of primary medical, dental, and behavioral health services. CCHS is a participating provider with most major insurance companies and HMOs. In addition, a sliding fee scale is available for any patient without insurance. As a community health center, CCHS is able to use federal funding to expand current services and develop new programs to meet the health care needs in our area. DYK? Each Choptank Community Health System School-Based Health Center is staffed by licensed, advanced practice clinicians, including Certified Nurse Practitioners and Physician Assistants.

School-Based Health Centers function like a typical doctor’s office, with medical providers assessing and diagnosing illness, writing prescriptions, performing lab tests—including for strep, influenza, and COVID—and physicals, including routine well visits and sports physicals.

Dental services include screenings, cleanings, sealants, fluoride treatments, and emergency visits, all of which are coordinated with your child’s primary dentist.

The easiest way to schedule an appointment is by calling the SBHC. Parents/families can also reach out to the school’s health staff for assistance.

(CCHS) is a private, non-profit community health center focused on providing primary medical and dental health services in Caroline, Dorchester, and Talbot Counties and the surrounding areas. CCHS health centers provide a full range of primary medical, dental, and behavioral health services. CCHS is a participating provider with most major insurance companies and HMOs. As a community health center, CCHS is able to use federal funding to expand current services and develop new programs to meet the health care needs in our area.

CCHS is a nationally recognized Patient-Centered Medical Home, delivering higher quality and more integrated care than many health systems throughout the United States. In September 2019, Choptank Community Health was awarded a Gold-level award by the American Heart Association for improving the health and well-being of patients with high blood pressure.
