United healthcare parents!

Attention Parents.

If you received a letter from UHC/United stating that we are no longer participating. THIS IS NOT TRUE.

This was an error on United’s end. Our contract was renewed on 11/1/2025 and they erroneously sent letters to our patients. Please ignore this notice. United said they would send a corrected letter but it doesn’t appear these were sent yet.

We may have to reassign Dr John Mulawka as your P*P.

Please call our offices with any questions or concerns.

To all our patients who currently use Rite Aid and are concerned about your prescriptions— there’s no need to worry. If you need to switch pharmacies, please contact our office with the name of your new preferred pharmacy.
Important insurance notes: CVS does not accept Independent Health. Walgreens does not accept Fidelis or Molina.
We recommend checking with your insurance provider to confirm which pharmacies are covered before calling us.
Once you’ve selected a new pharmacy, we will send over your medications when they are due for a refill. Please note: we do NOT send your entire medication list all at once, medications are transferred individually at the time of refill to help ensure accuracy and avoid errors.

Change to our schedule for next week! (Derby office does have normal hours)

We have changes to our office schedules for next week Easter Break!

Abbott Road Office: Open Monday 4/14 and Wednesday 4/16. CLOSED Tuesday 4/15 Thursday 4/17 and Friday 4/18. Please note that there will be NO ACCESS to the office on Thursday and Friday as the driveway is getting redone and extended.

Dunkirk Office: Open Monday 4/14 and Friday 4/18. CLOSED Tuesday 4/15 Wednesday 4/16 and Thursday 4/17. Please note that there will be NO ACCESS to the office from Tuesday-Thursday as the roof and gutters are being repaired.

Silver Creek: Open Tuesday 4/15 and Thursday 4/17. CLOSED Monday 4/14, Wednesday 4/16 and Friday 4/18.

Big Tree: Closing at 1pm on Tuesday 4/15. Otherwise, Monday, Wednesday, Thursday and Friday are normal office hours.

Derby: No change to the schedule. Open normal office hours including Saturday sick clinic on 4/19.
