The Iowa Primary Care Association has been selected by the Iowa Department of Health and Human Services Iowa Department of Health and Human Services to serve as the statewide Communities of Care Technical Assistance Provider as part of Iowa's Rural Health Transformation Project and the Healthy Hometowns initiative.

Through this work, the Iowa PCA will provide training, tools, and implementation support to help rural Iowa providers and community organizations develop sustainable co-located Communities of Care.

In recognition of Mental Health Awareness Month, we’re sharing real examples of how System Navigators help Iowans access mental health support.

Recently, a telehealth organization contacted our System Navigation team after a parent shared concerns about their child’s behavior and mental health.

A System Navigator connected the family to Behavioral Health Intervention Services (BHIS), psychological testing, community resources, and a Child Mental Health Wraparound program. They then successfully assisted the family in applying for a Child Mental Health Wraparound program. These services are now in place, and the child is receiving the needed care.

May is Mental Health Awareness Month. Our System Navigation team connects Iowans to support and resources for social needs that impact health and well-being—including behavioral health. This month, we’re sharing real examples of how System Navigators support Iowans seeking mental health resources, like the story below.

A staff member from a local aging and disability organization reached out to our System Navigation team for help securing transportation for a client’s mental health appointment.

A System Navigator not only arranged the ride but also partnered with a local church coalition to cover the cost, removing another barrier to care for the patient.
