Snowline Health Snowline's mission is to bring peace and dignity through life’s transitions by providing comfort, care, and support to people in need.

Snowline, established in 1979, is a nonprofit, community-based organization dedicated to to serving our community by enhancing the lives of those with chronic, serious illness, and patients nearing end-of-life. The care provided to patients and families is carried out by a Snowline Interdisciplinary team of professionals that includes physicians, registered nurses, licensed vocational nurses, medical social workers, spiritual care providers, hospice aides and volunteers. The majority of Snowline's dedicated staff has earned the distinction of "Certified in Hospice and Palliative Care," an advance level of education. Hospice benefits are included in Medicare and Medi-Cal and most health insurance plans. The uninsured and palliative care recipients are also served at Snowline through funds received from the proceeds of Snowline's six thrift stores, a recycle and repurpose processing center enterprise, individual donations, fundraisers, and the United Way. At Snowline, neither patient nor family will ever receive a bill for services. We serve the western slope of El Dorado County and the Greater Sacramento Region: including Sacramento, Folsom, Roseville, Granite Bay, Fair Oaks, Carmichael and Rancho Cordova.

On May 7th, we invite our community to come together for Big Day of Giving, a day dedicated to supporting local nonprofits like Snowline.

For over 46 years, Snowline has been honored to serve individuals and families facing serious illness, grief, and loss. Every day, our compassionate team provides care that brings comfort, dignity, and support when it matters most.

Your gift on Big Day of Giving helps make this possible by supporting:

- Palliative care for serious illnesses including cancer, COPD, Alzheimer’s, dementia, and heart disease
- Grief counseling for children, adults, and families
- Veteran services that honor and support those who served
- ElderCare program provides compassionate, in-home support for older adults managing chronic illness or age-related needs, helping maintain comfort, dignity, and quality of life, and includes eldercare physicians who can serve as a patient’s primary care provider while coordinating their overall care.

Every dollar stays right here in our community, helping ensure no one faces these moments alone.

The Hospice Care team is an interdisciplinary group of individuals who work together to meet the physical, medical, psychosocial, emotional, and spiritual needs of the hospice patient and family. Each team member brings specialized expertise, but all share the same goal: comfort, dignity, and respect.
An interdisciplinary team can include the Attending Physician, Medical Director, Nurse Practitioner, Registered Nurse, Medical Social Worker, Spiritual Care Provider, Bereavement Coordinator, Home Health Aide, and Trained Volunteers.
