Severe Hypoglycemia Linked With Over-the-Counter Supplement

An over-the-counter supplement promising male sexual enhancement led at least 17 men in Virginia to develop severe hypoglycemia in 2019, the CDC reported.

The first case, a 57-year-old man without diabetes, presented to an emergency department with profuse sweating, increasing agitation, and confusion. His blood glucose was 48 mg/dL, well below the normal range of 70 mg/dL to 100 mg/dL. The man said he used an over-the-counter sexual enhancement supplement after being unable to fill his sildenafil prescription.

Emergency medical services treated a second man, aged 50 years, also without diabetes, who was sweating, trembling, and confused. His blood glucose level was 32 mg/dL. He reported taking the same supplement and was hospitalized for 3 days. Virginia public health authorities identified 15 more hypoglycemia cases associated with the pills. All the men were treated with intravenous dextrose; 7 also received octreotide.

State authorities removed the supplement, marketed as V8, from 23 locations in Virginia after testing revealed that it contained 55 mg to 156 mg of sildenafil per tablet—roughly a therapeutic dose. Most tablets also contained 90 mg to 100 mg of glyburide, a sulfonylurea used to treat type 2 diabetes.

“Numerous tainted sexual enhancement products remain on the market as over-the-counter products, placing consumers at risk for unknown health complications,” the investigators wrote.
