The Medical Center of Dimmitt is taking applications for a Chronic Care Coordinator. Qualifications for this position include RN who has clinical experience working with medically complex/chronically ill patients and their family/caregiver(s). Other qualifications include strong communication skills and a compassionate approach to care. Bilingual is also preferred.
RN will be responsible and accountable for the coordination and management of direct and indirect patient care that utilizes expertise in the nursing process, as well as critical thinking skills. The Chronic Care Coordinator collaborates and partners with the patient, patient’s family/caregivers, and the healthcare team to prevent any gaps in care and monitor patient progress. This RN must also be open to training in diabetes management.

PT or PTA degree from an accredited program
Valid Texas license (or pending)
Current BLS certification

Schedule: Monday–Friday, 8am–5pm (occasional weekends/holidays)
