A Nurse in the Family — Bridging the Gap in Care. 💙
A family reached out to us recently because their mom just seemed "a little off." They couldn't quite put their finger on it — but something didn't feel right.
We stopped by for a visit.
She seemed okay at first — until she told us it was the year 1919. And when we asked her to stand and walk, she could only manage 3 steps.
To a family, that might be confusing or easy to dismiss. To a nurse, those are immediate warning signs.
She was sent to the emergency room that day. What followed was a 5-day hospital stay and a transfer to a skilled nursing facility for rehabilitation.
Here's the thing about early intervention — it means catching problems before they have a chance to spiral. In older adults, what looks like a small issue can turn serious very quickly. The sooner the right care begins, the stronger the recovery and the lower the risk of ending up in a much worse situation.
But our job didn't stop at the ER. 👇
Once she was settled into the skilled nursing facility, we showed up to her first care conference. That's the meeting where the medical team reviews her test results, her progress, and the plan for getting her home.
We reviewed her blood work. Two standard tests had been ordered — done promptly and appropriately. But magnesium levels had not been checked.
Why does that matter? Because the entire time she was in the hospital, the medical team had been giving her magnesium to replace what her body was missing. Low magnesium causes fatigue and muscle weakness — which was a big part of why she could barely walk when we first saw her.
The lab test to check if her levels were recovering? Not ordered.
Who thinks to check for that? We do. 💡
We flagged it. The staff added the test. Simple as that — but only because someone was in that room who knew what to look for.
Now imagine if no one had caught it. Her magnesium stays low. She gets too weak to do therapy. And instead of heading home — it's back in the ambulance, back to the hospital.
One missing lab test. Potentially a whole new hospitalization.
This is the reality of healthcare today. The teams are hardworking and doing their absolute best — but the system is complex and the pace is fast. Things get ordered, but not always ALL the right things. Details slip through — not out of carelessness, but because no one person can catch everything.
