A Nurse in the Family — Bridging the Gap in Care. 💙

A family reached out to us recently because their mom just seemed "a little off." They couldn't quite put their finger on it — but something didn't feel right.

We stopped by for a visit.

She seemed okay at first — until she told us it was the year 1919. And when we asked her to stand and walk, she could only manage 3 steps.

To a family, that might be confusing or easy to dismiss. To a nurse, those are immediate warning signs.

She was sent to the emergency room that day. What followed was a 5-day hospital stay and a transfer to a skilled nursing facility for rehabilitation.

Here's the thing about early intervention — it means catching problems before they have a chance to spiral. In older adults, what looks like a small issue can turn serious very quickly. The sooner the right care begins, the stronger the recovery and the lower the risk of ending up in a much worse situation.

But our job didn't stop at the ER. 👇

Once she was settled into the skilled nursing facility, we showed up to her first care conference. That's the meeting where the medical team reviews her test results, her progress, and the plan for getting her home.

We reviewed her blood work. Two standard tests had been ordered — done promptly and appropriately. But magnesium levels had not been checked.

Why does that matter? Because the entire time she was in the hospital, the medical team had been giving her magnesium to replace what her body was missing. Low magnesium causes fatigue and muscle weakness — which was a big part of why she could barely walk when we first saw her.

The lab test to check if her levels were recovering? Not ordered.

Who thinks to check for that? We do. 💡

We flagged it. The staff added the test. Simple as that — but only because someone was in that room who knew what to look for.

Now imagine if no one had caught it. Her magnesium stays low. She gets too weak to do therapy. And instead of heading home — it's back in the ambulance, back to the hospital.

One missing lab test. Potentially a whole new hospitalization.

This is the reality of healthcare today. The teams are hardworking and doing their absolute best — but the system is complex and the pace is fast. Things get ordered, but not always ALL the right things. Details slip through — not out of carelessness, but because no one person can catch everything.

Then it was straight over to the Sensational Seniors Workshop, where I joined over 80 vendors to represent Nurse in the Family and spread the word that HELP IS OUT THERE for our seniors and their loved ones. 💙
It truly lights my heart up to connect with seniors and families who may not know what resources and support are available to them. That's exactly why we do this work.

Such a wonderful afternoon! 🌟
Today, Susan and I had the opportunity to speak with Mount Diablo Village members about some really important topics - fiduciaries, estates and trusts, annual healthcare planning, and prevention for stroke and heart attack.

But we also talked about the everyday things that matter just as much: What happens if you fall and fracture your wrist? Who would you call? Who could pay your bills? Who could take you to doctor's appointments when you really need someone?

Here's the truth: Aging doesn't have to be scary.

It's all about having a plan so you're not facing disjointed care when consistent support matters most. And there are SO many good quality people and services out here focused on seniors, especially within Contra Costa County.

We're incredibly grateful to Ellen Osmudson for having us at Mount Diablo Village.

Hopefully some of what we discussed resonated with their members and helps them feel more prepared for whatever comes next. 💙

This is a PERFECT example of why hiring a third party to review your medical records and history is so important.

Too often, people assume "my healthcare team will take care of me." But the reality? Your care is NOT personalized. It's checking boxes with no additional critical thinking—limited to what they offer and who they partner with.

The Reality:
I am reviewing a case for a senior currently falling multiple times weekly with sustained injuries. Kaiser Permanente sent Home Health and PT, then discharged him with this note:

"Discharge - No Further Home Health Care Needed"
"Continued needs identified: none"
No discharge instructions. No community resources. No follow-up plan.

Meanwhile, an independent assessment revealed:

✅ Immediate Safety Needs:

Current 2-wheeled walker catching on rugs (causing falls)
Missing bed rails for safe transfers
Unsecured multiple throw rugs creating trip hazards
Improper footwear increasing fall risk
Review of medications causing balance and safety issues
Connection with placement specialist for respite

✅ Home Modifications Needed:

Grab bars in multiple bathrooms
Proper lighting and clear pathways
Carpet securing and hazard removal

✅ Ongoing Support Required:

In-home physical therapy for strength and balance
Caregiver support 4-8 hours/week
Fall prevention equipment (rollator walker, bed rails, possibly protective airbag vest)

The takeaway?
One system said "you're fine, no further care needed."

An independent review uncovered critical safety gaps that could prevent the next fall, the next head injury, the next hospitalization.

Yesterday I had one of those moments that stops you in your tracks.
I walked out to get the mail just as the carrier was making her way to my house. I'd never met her before, though I've gotten her timing down pretty well. When she paused at my mailbox, I asked if she had anything for number 27. She just looked at me for a few seconds. Then she said something that caught me completely off guard: "I recognize your voice."
Turns out her husband had been my patient, and she remembered me. She remembered how I advocated for him and helped get him the care he needed after a critical injury.
These moments happen maybe once a year—running into someone in the community, now apparently at my own mailbox.
They're reminders that people remember how you treated them. That showing up with genuine care, making sure someone feels heard and safe, educating them through whatever they're facing—it matters. It sticks with people long after the encounter ends.
It's easy to forget that in the day-to-day. But yesterday, standing at my mailbox, I was reminded: presence matters. Advocacy matters. People remember.
