Many times the arrangement of teeth in the mouth will not allow for the ideal restoration. When you have lower teeth that are not in line and invade the space of the uppers, (and vice versa), you have to work around them. This paves the way to make a somewhat abstract restoration which tends to look very natural. This case is a three unit implant bridge with a single crown on the other Central. The lingual / inside is in metal to protect the porcelain when the patient goes into lateral excursions, (side to side movement). The holes in the metal are for the screws that hold this in place. This is called screw retained. The doctor will cover them after it's inserted, and if anything ever goes wrong, the Dentist can remove it by unscrewing it.
