Mrs. F. arrived at Heritage Pointe requiring significant assistance for bed mobility, transfers, and walking.

Our therapy team focused on these core functional areas to help her regain her autonomy. By discharge, she had achieved complete independence in self-care activities and was able to manage her own mobility with the assistance of a rolling walker. We are thrilled to see her return home safely. 👏
