Due to improper oral hygiene during orthodontic treatment, this young client experienced significant dental decay and enamel weakening.
Traditional dentistry typically offers two main options in such cases: either removing and damaging more tooth structure by placing crowns or taking a more conservative approach by patching decayed areas. The latter, while preserving more of the tooth, can lead to aesthetic issues and/or not even address other aesthetic concerns of the client.
Fortunately, this client did their research and discovered that we provide an alternative that achieves aesthetic enhancement without unnecessary removal of tooth structure. Our approach not only addresses the decayed areas but also strengthens and seals the teeth, improving overall longevity and reducing the risk of further tooth structure loss. In the end, we successfully treated all the decayed areas, fortified and sealed the teeth, and enhanced the client's smile.
Executing this skill is not easy! There are certainly simpler ways to complete this treatment, such as grinding down teeth for crowns or opting for aggressive veneers. However, would these methods be better for the client? We don’t believe so. Our method ensures the best outcome for the client's dental health and appearance.
CASE
After removing all the interproximal decay, the teeth were properly conditioned, and their cores were built with DA2 Estelite Omega using Bioclear Matrices (A103). The incisal edges of all teeth were simultaneously built using a matrix and White Occlusal Microhybrid Cosmedent for added strength.
The entire facial surface, two teeth at a time, was built with SB3 Microfill from Cosmedent, utilizing Unica metal matrices. The shapes were carefully sculpted and "cut back," and tints (Ochre, Gray, Blue, and LO+ White from Tokuyama) were applied. Mamelons were created with BL2 Estelite Omega. A final enamel layer of Light Incisal Microfill (Cosmedent) was placed on the facial surface.
Finishing, texture creation, and polishing were performed by Dr. Pessoa's sequence. with tissues isolated using Ultradent 00 cord, Isolite, and Optragate.
