At The Chautauqua Center, we're here to help you check off those back-to-school to-do's. School physicals are a great way to ensure your child is healthy and ready to learn. Plus, staying current on vaccinations helps protect your child, their classmates, and our community from preventable diseases.

We are still taking all patients and prescriptions at the Waterfront Pharmacies in Dunkirk and Jamestown, as well as our South Dayton Pharmacy. The South Dayton Pharmacy serves the Gowanda area in partnership with Tri County Family Medicine. We deliver free to these areas, including Cherry Creek and Conewango. >>> Our Waterfront Pharmacies in Jamestown and Dunkirk are experiencing a server outage. This affects the ability of our pharmacists to access refills, fill any scripts, or check anything on patient pharmacy profiles. We ask that you please be patient while our professionals work hard to bring the servers back up.

This server outage is also impacting some services at our medical and physical and occupational therapy locations.

South Dayton Pharmacy is not affected by this server outage.

National Health Center Week was a huge celebration of company and community pride! We had a number of staff activities and theme days in recognition of our health centers and our various teams. There was 90s Hip Hop Day, Character Day, Throwback Day, Ice Cream Truck Days, International get togethers, and sporting our 2025 NHCW t-shirts! We had a blast and hope you get a bit of joy our of our celebration!

Founded in 2013, the Chautauqua Center is a Federally Qualified Health Center and has been recognized as a leader in the provision of health care services in both Dunkirk and Jamestown. In 2017, The Chautauqua Center proudly achieved the designation of a Patient Centered Medical Home (PCMH), reflecting its excellent track record of providing advanced primary care by taking the extra step for patients and those in the community. As a PCMH, the Center serves as a one stop shop that offers a large variety of services under one roof. This allows for improved communication among providers, streamlined services and greater access to care.
