An intensive clinical and functional face-to-face evaluation by a Licensed Professional of an individual’s presenting mental health, developmental disability, and/or substance abuse conditi
on that results in the issuance of a written report, providing the clinical basis for the development of a Person-Centered Plan [P*P] and recommendations for services/supports/treatment. Peer Support Services (B3) (Alliance)
Services are structured and scheduled activities for adults age 18 and older with MH/SA disability. Peer Support service is an individualized, recovery focused approach that promotes the development of wellness self-management, personal recovery, natural supports, coping skills, and self-advocacy skills and development of independent living skills for housing, employment and full community inclusion. OUTPATIENT TREATMENT (THERAPY)
Outpatient Treatment is designed to meet the clinically significant behavioral or psychological symptoms or patterns that have been identified as treatment needs for a recipient. It is provided through scheduled therapeutic treatment sessions and may be provided to individuals, families, or groups in various settings. INTENSIVE IN HOME
Services delivered to children and adolescents, primarily in their living environments, with a family focus, to: provide parenting skills and training that builds effective communication and positive behavioral intervention strategies with the youth to help meet the Person Centered Plan goals; provide therapy with the youth and family; work with the youth and their caregivers to implement home-based behavioral support plans; provide ongoing evaluation of the presenting psychiatric or addiction symptoms; identify strengths that will aid in stabilizing the family unit as well as identify barriers that may impede the development of skills necessary for functioning in the family and community; provide psychoeducation to the youth, family and community; defuse current crisis, evaluate its nature, and intervene to reduce the likelihood of a recurrence; ensure linkage to needed community services and resources. Durham Public Schools MH Co-located Services
Outpatient therapy clinic housed in select Durham county elementary schools which offer an on-site licensed therapist who specializes in child mental health. Services include: comprehensive clinical assessments, individual, family, and group therapy during school hours to students within these schools (C.C. Spaulding Elementary, Club Blvd. Elementary, Hope Valley Elementary, Parkwood Elementary, Lowes Grove Middle, Rogers-Herr Middle, Shepard Middle, Durham School of the Arts, and Riverside High). Psychiatric medication management and Psychological evaluation also offered to students enrolled in this program. In addition, therapists collaborate with school teachers and staff within the schools to help students in the program achieve greater academic and behavioral success in the school setting. Students can be referred to the program by school support team or by parents who are concerned about a child's emotional or behavioral health. MEDICATION MANAGEMENT
Provide medication screening and education for consumers; facilitation of safe and effective use of prescription drugs. Individual Supports (B3) (Sandhills)
Individual Support is a “hands-on” service for persons with Serious and Persistent Mental illness (SPMI), a population. The intent of the service is to teach and assist individuals in carrying out IADLs, such as preparing meals, managing medicines, grocery shopping and managing money, so they can live independently in the community. It is envisioned that the need for the service will “fade” or decrease over time as the individual becomes capable of performing some of these activities more independently.
