This session provides an in-depth, practical update on direct oral anticoagulant (DOAC) management, with a focus on complex clinical decision-making in high-risk patient populations. Participants will explore current evidence, safety considerations, and future directions in anticoagulation therapy.

Key discussion points include the use of DOACs in end-stage renal disease, clinically significant drug–drug interactions, and when—and how—to appropriately interpret DOAC concentration testing. The session will also compare bleeding risk assessment tools and examine emerging anticoagulants, including Factor XIa inhibitors, with a review of current phase 2 and phase 3 clinical trial data and potential future clinical applications.

This session will provide an overview of the North Carolina Rural Health Transformation Program, including its structure and key initiatives, with a focus on opportunities most relevant to pharmacists.

Attendees will explore how pharmacists can play a vital role in improving care delivery in rural settings—helping to reduce the burden of chronic illness, improve acute care outcomes, and strengthen access through new models of care such as value-based care and capitation. The discussion will also highlight how pharmacists can collaborate with interdisciplinary teams to advance health equity and improve outcomes across the state.

Led by Lawrence Greenblatt, a physician leader with extensive experience in public health, Medicaid policy, and statewide health initiatives, this session will offer a meaningful look at the future of rural health in North Carolina.
