"I established care with Dr. Dennis Jacokes at Imperial Center Family Medicine in 2001 and remained his patient until his retirement in 2024. Rather than switching to a provider closer to home, I chose to stay with ICFM given my long history there and was assigned to PA Kaitlin Deslauriers.
