In part two of our sessions with Bob, I continue my work to help him build the courage to face what he fears most. Bob’s anxiety stems from a deep need to avoid getting “stuck” in spiraling thoughts. But what if the path to freedom lies in doing the exact opposite?

In this second session, I call on Bob’s natural resilience. I teach him to actively approach anxiety-provoking situations, using powerful analogies that reframe the fear and energize the fight.

Severe OCD can feel like an endless battle—especially when previous therapy hasn’t helped. In our first of two sessions, we meet Bob, a personal trainer who’s struggled with OCD since childhood. Now, in what he calls the worst period of his life, Bob is overwhelmed—avoiding triggers, losing sleep, and falling behind in both his personal and professional goals.

Despite months of prior exposure-based therapy, Bob hasn’t found relief. But in just one session, we begin to shift that narrative. In this video, I introduce key concepts for treating anxiety disorders—laying the groundwork for true change.

Therapeutic resistance is a familiar challenge, but ambivalence in clients with OCD presents a uniquely complex barrier to progress. In the video below, I demonstrate how to shift this dynamic using a bold, counterintuitive cognitive-behavioral approach.

If you saw our last video about Mary—an avid traveler who struggles with intense claustrophobia—you might remember how we worked on two transformative therapy sessions. Her fear of environments she feels are inescapable has been panic-inducing for her throughout her life.

I'd like to share with you a follow-up video where Mary and I discuss her experience facing her fear directly and the remarkable results she experiences.

Mary's achievement after just two sessions is exceptional.

"I turned the radio off and kept the windows up, which I never do," she states. "I wanted to get that panicky part of my brain going."

Without medication, Mary has taken significant steps toward control over her panic attacks, improving her quality of life.

It’s part of a rare, real-time look at the principles of exposure therapy that can help mental health professionals looking to deepen their understanding of treating claustrophobia, panic, and avoidance behavior. Here you’ll see how you might conduct and then debrief an interoceptive exposure to suffocation.

In the full video, you'll observe me guiding a client, Mary—an avid traveler who struggles with intense claustrophobia—through two transformative therapy sessions. Mary’s fear of suffocation and entrapment extends beyond flying; she avoids elevators, parking structures, and any environment that feels inescapable. When avoidance isn’t an option, she battles severe panic attacks.

Through carefully structured cognitive and behavioral experiments, I help Mary confront these fears head-on. From breathing through a nose plug, to sealing her head inside a pillowcase, and even stepping into a sealed cardboard box, Mary engages in powerful exposure exercises designed to reframe her experience of fear.

Panic disorder can be a daunting challenge—not just for those who suffer from it, but for the clinicians who treat it. A key component of the disorder is the fear of future panic attacks, leading to avoidance behaviors that reinforce anxiety. While Exposure Therapy is considered the gold standard treatment, many therapists have never had the opportunity to see it in practice—until now.
