Lethargy, sunken eyes, dry mucous membranes, and reduced urination.

History: 🥵
Reports decreased water intake and possible fluid loss from sweating over the past 5 days, likely due to heat stress and inadequate access to water.

Physical Examination: 🩺
General Condition: Lethargic, decreased responsiveness
Hydration Status: Sunken eyes, dry oral mucosa

Vital Signs:
Heart rate: Elevated
Respiratory rate: Increased
Temperature: Elevated

Assessment:
Moderate dehydration likely secondary to heat exhaustion and reduced water intake

Treatment Plan:
Oral Electrolytes
Cooling Measures

Prognosis:
Good with prompt and adequate rehydration and resolution of underlying cause.
