Nervous system sensitive clients aren’t “resistant.”
They’re responding to something we may not even realize we’re communicating.

As clinicians, we’re trained to focus on interventions like CBT strategies, treatment plans, measurable outcomes.

But here’s the shift that has changed everything in my work:

👉 Before cognition, there is neuroception.
👉 Before insight, there must be safety.

Many of our clients, especially autistic individuals and those with trauma histories, are constantly scanning for cues of safety or threat.

Not consciously.
Not cognitively.
But physiologically.

Which means…

Your client is not just listening to your words.

They are reading:
Your tone
Your pacing
Your body language
Your proximity
Your energy

You are the intervention before any intervention begins.

What this looks like in practice:

When a client shuts down, avoids, or escalates…

Instead of asking:
❌ “Why aren’t they engaging?”

We shift to:
✅ “What is their nervous system detecting right now?”

Real Clinical Shift (That Works):
✔ Lower your voice
✔ Slow your pace
✔ Reduce movement
✔ Soften facial expression
✔ Sit at an angle instead of directly facing
✔ Use fewer words
✔ Pause more than you think you need to

This isn’t about doing less.
It’s about reducing perceived threat so the brain can re-enter a learning window.

Because when the nervous system is in defense:
Executive functioning goes offline
Insight doesn’t land
Skills don’t stick

The truth we don’t talk about enough:
Co-regulation is not a technique.
It’s a state.

If we are:
Rushed
Outcome-driven
Dysregulated ourselves

Our clients feel that, whether we name it or not.

A simple reframe I come back to often:

“Am I creating a space where this nervous system can feel safe enough to learn?”

Not perfect.
Not fully open.
Just… safe enough.

Quick Clinical Reminder:

➡ Safety > Insight > Intervention
➡ Regulation precedes cognition
➡ Small shifts in your presence create big shifts in engagement

This is the work.

Instead of focusing first on behavior or compliance, this perspective prioritizes nervous system safety, co-regulation, and access to the prefrontal cortex. When a client is regulated, they are more able to engage, communicate, and benefit from therapeutic support.
