If you plan to book an appointment for another person I also need all of THEIR information, I will NOT accept clients using their information for someone else getting an appointment. New York State requires ALL clients to have an intake form for my records and I require ALL clients to fill one out PRIOR to your appointment. I appreciate you, thank you!!
