As we start 2025 we would like to remind all patients of our policies that will be effective starting today,

✔️Bring your insurance card to every appointment
✔️Get ALL refills at your appointment or you will have to see the doctor again
✔️Copays are due at the time of the visit
✔️All past due accounts must be paid on at the time of service and a payment plan set in place.

✔️All balances must be paid off within 6 months.

A copy of the new policies can be found at the front desk.
