Hope Creek Nursing and Rehabilitation Center is a 245 bed skilled nursing facility, with private and semi-private rooms available for short-term skilled stays, respite stays, and long term care. We accept Medicare, Private Pay, VA Contracts, Medicaid and "most" managed care insurance plans.

Residents enjoyed choosing their favorite flavors and toppings, sharing stories, and soaking up time together. These simple, joyful moments do more than satisfy a sweet tooth, they help spark memories, encourage social connection, and provide comforting routines that are so important for those living with memory loss.

Taco Tuesday turned into a full-on fiesta in yesterday’s Cooking Class! 🌮

Our Hope Creek chefs built their own taco salads, loaded with all the fixings and plenty of smiles. Residents had fun choosing their toppings, visiting with friends, and...of course...taste testing their delicious creations.

Huge thank you to our staff who helped prep ingredients, plate, and join in on the fun. We love seeing our residents stay engaged, try new activities, and enjoy good food together!

She leads the specialized neighborhood that serves residents living with Alzheimer’s disease and other dementias, guiding staff in providing patient, respectful, and person‑centered care. Lydia works closely with families, nursing, activities, and therapy to create structured routines, meaningful engagement, and a safe, calming environment tailored to each resident’s abilities.

The Alzheimer’s Unit Coordinator role is especially important because dementia care requires consistent approaches, specialized training, and a deep understanding of behavior and communication changes. Our Hope Creek team has a lot to celebrate! Over the past several months, our staff has worked incredibly hard to improve the quality of care we deliver every day, raising the bar for safety, service, and resident satisfaction.
