Continued access to affordable assisted living and supportive housing is a critical necessity for those that Aldersbridge Communities serves. They are aging in place with increasing frailty; have multiple medical conditions; failing vision and hearing; and impaired mobility -- all of this compounded by impoverishment. Winslow Gardens, located in a residential neighborhood in East Providence, RI, offers a combination of independent-living subsidized apartments and affordable assisted living options for low to moderate income individuals 62 years or older. They have efficiency units as well as one-bedroom units that can accommodate one and two person living arrangements. Living options are complemented by 24-hour emergency response, full-service dining, and assistance with activities of daily living including housekeeping, laundry assistance, personal assistance, nutrition and hydration monitoring, medication reminders, mobility assistance, transportation services and planned activities and outings. Services are complemented by a continuum of care that includes skilled nursing, recuperative therapies, and long-term care. At Linn Health & Rehabilitation, they offer skilled nursing care, various levels of post hospital care, on-site rehabilitation as well as short-and-long-term medical care in a private or semi-private room in a 73 bed extended care facility. Services at Linn Health & Rehabilitation are designed to identify and address core issues surrounding the social, physical and psychological needs of an individual, enhance and maximize individual functioning and independence, and provide flexible treatment options to cater to individual needs. Services include post-op rehab care, skilled nursing care, hospice services, short-and-long-term care, respite stays, on-site rehabilitation services including physical, occupational, and speech therapy, social service support, transport services and spiritual care. Arbor Hill Assisted Living, Aldersbridge Communities newest facility, is located in the heart of historic Federal Hill area in Providence and provides housing and services for seniors of modest means. It is a fully licensed assisted living facility in the state of Rhode Island and is a member of the Rhode Island Assisted Living Association and LeadingAge RI. It offers assisted living facilities with housing units for one to two persons with full dining, personal care and medication assistance, health services, housekeeping, laundry and transportation as well as a wide-variety of planned activities and outings. Services at Arbor Hill are provided with the goal of giving residents the opportunity to maintain independence with dignity and respect in a caring community, in a place that truly feels like home. At all three subsidiary organizations, an on-site chaplain and dynamic activities staff work to create a warm, enriched, and inviting community life with financial assistance and subsidies available for those living on modest means. Aldersbridge Communities in a commitment to ensure organizational financial stability, sustainability and revenue diversification for its future and the growing need for services has recently established a for-profit subsidiary, Elder Care Consultants. Elder Care Consultants is a licensed home care provider with extensive collective experience in providing comprehensive care and services to individuals in their own homes. As a member of Aldersbridge Communities, Elder Care Consultants staff recognizes and values the importance of the continuum of care through the aging process and has the knowledge and expertise to effectively match needs with services to ensure that each individual receives the exact services required to age gracefully and productively in their own homes and communities. Ala carte services are designed to provide each individual with the optimum living environment and level of care that best suits specific needs including skilled nursing services, certified home health aides, homemaker services, companion services, 24-hour emergency coverage, personal care assistance, meal preparation, nutrition and hydration monitoring, medication reminders, mobility and ambulance assistance, light housekeeping, laundry assistance and dementia care. Most recently, Elder Care Consultants LLC, a division of Aldersbridge Communities, now manages, St. Germain located at 429 East School Street in Woonsocket owned by the Woonsocket Housing Authority. This is a model assisted living program, the first of its kind in which a housing authority as a HUD-governed high rise, is being partially converted to assisted living with nursing and support services being offered to tenants by Elder Care Consultants LLC. Germain in Woonsocket, it offers assisted living facilities with housing apartments for one to two persons with low to moderate incomes including with full dining, personal care and medication assistance, health services, housekeeping, laundry and transportation as well as a wide-variety of planned activities and outings. Germain are provided with the goal of giving residents the opportunity to maintain independence with dignity and respect in a caring community, in a place that truly feels like home. A professional staff, personalized care plans and strong emphasis on communication mean high-quality care that respects each resident's individual needs and wants. Every aspect of St. Germain, including the physical facility, staff selection, care levels and activities has been carefully selected and reviewed to help our residents maintain an active and engaging lifestyle. The Aldersbridge Navigator provides resources and support to elders who have been discharged from a nursing home or hospital and who live independently in their own homes. The Navigator follows a resident offering resources for medical care, therapies, pharmacy, heating and food among others. The goals of the Navigator program are to keep elders living safely and independently in their own home and reducing ER visits to the hospital and or readmittance to a nursing home.
