CNY Family Care takes a team approach to meeting the medical needs of our patients through compassion, respect, and professional care. We are available for our patients when they need us. We partner with our patients to provide a proactive approach to medical care, which improves their quality of life.

We are a Family Oriented Medical practice with Physicians, Physician Assistants, Nurse Practitioners and Diabetic Educators. All of our physicians are Board Certified by the American Board of Family Medicine, Academy of Family Physicians. Our Nurse Practitioners and Physician Assistants are individually board certified and licensed by the State of New York. They work directly with our physicians to provide all of our patients with the best family care possible.

One of our main goals at CNY Family Care, is to provide all of our patients with continuous and comprehensive health care. As part of this care model, we ask all of our patients to select one personal physician. This PCP/Primary Care Physician will maintain an ongoing relationship with you and will then always be in charge of your care. They will be in charge of the care team of nurses, Nurse Practitioners, Physician Assistants and Diabetic Educators who might interact with you when you visit the office. Our care team will work towards ensuring that you receive the appropriate age related preventative testing and treatment as well as the latest evidence based treatment (proven and documented) for your individual medical condition(s).
