📣 Now Offering Eversense 365 at Lake Country Medical Group

We are excited to share that we are now offering Eversense 365, the first and only implantable continuous glucose monitor designed to last up to one full year.

So what is Eversense 365?

Eversense 365 is a small sensor placed just under the skin that continuously monitors glucose levels day and night for up to 12 months. Unlike traditional CGMs that require frequent sensor changes, Eversense is inserted once and provides long-term, real-time glucose data straight to your smartphone.

Here’s what that means for you:
✔️ Continuous, real-time glucose readings
✔️ Fewer sensor changes
✔️ Alerts for high and low glucose levels
✔️ Greater convenience and confidence in daily diabetes management

Eversense 365 offers a new level of freedom and consistency in glucose monitoring.

Many students in our community rely on free or reduced school meals during the school year, and summer can create a real gap for families in need.

The Greene County Food Pantry is hosting the “Feed the Need” Food Drive on Saturday, June 6th, and every donation can make a difference for local families right here in our community.

As we reflect this Memorial Day, one of our patients, Tim Cadiere, shared this original song with us that felt especially meaningful for the occasion. 🇺🇸

Today, we remember and honor the brave men and women who gave their lives in service to our country, along with the families who carry their memory forward.

Thank you, Tim Cadiere Music, for being a part of and sharing your music with our community.

Magnesium plays a role in hundreds of processes throughout your body, yet many adults may not be getting enough of it.

Low magnesium levels can sometimes contribute to symptoms like:
• Fatigue
• Muscle cramps
• Poor sleep
• Headaches
• Stress and tension

In this new blog from Dr. Nicolas Chronos, Karen Schwartz, WHNP-BC, and Dr. Phoebe Sahawneh, PharmD, our team discusses why magnesium matters, why deficiency is more common than many people realize, and what you should know about supplementation and overall wellness.

✔️ Direct access to your care team

The goal is simple: better outcomes, fewer hospital visits, and greater peace of mind.

The best part? Chronic Care Management is covered by Medicare. Our team can help determine eligibility and explain any potential costs upfront.

💊 Dr. Peter Chapman on Supplements: What You Should Know 💡

In this video, Dr. Peter Chapman explains the importance of understanding how supplements can impact your overall health. While most supplements may not harm you, they could interact with medications for conditions like high blood pressure or diabetes, potentially affecting your treatment.

Dr. Chapman emphasizes the importance of discussing supplements with your healthcare provider to ensure safety and proper dosing—especially for vitamins like D, where blood work can guide recommendations.

If you have questions about your supplements or medications, call us at 706-485-4004 or visit www.lakecountrymedicalgroup.com. We're here to help!

Strong bones are built over time… and so is the conversation around protecting them. 💙

Osteoporosis is often called a “silent disease” because many women do not realize bone loss is happening until a fracture occurs. That is why early awareness, prevention, and screening matter so much.

We recently sat down with Dr. Eric Silver and Karen Schwartz, DNP, WHNP-BC to talk about women’s health, osteoporosis, what signs to watch for, and how women can take proactive steps to protect their bone health for the future.

If you have ever wondered when to start thinking about bone health, this is a conversation worth reading.
