Great call from Dr. D who identified an inflamed/unstable abdominal aortic aneurysm on unenhanced CT. These findings were confirmed on CTA.

[Images] Axial unenhanced CT images demonstrate indistinctness of the wall of the upper AAA with endograft in place. Delayed axial CTA image reveals an inflamed/enhancing aortic wall (red arrows), which is potentially infected. Additionally, an endograft leak is evident on the arterial phase (red circle) with contrast filling the aneurysm sac on delayed imaging (curved red arrow).
