📄 Provide proof of at least 5 off-island appointments for the same condition in the next 12 months
📄 Submit either:
-An official printed schedule from your provider, OR
-A signed letter on official letterhead from your provider
📄 Ensure all documents are professional, complete, and unaltered
🗓 Reduced rates last until your last listed appointment (up to 1 year). Renew annually with new documentation.

📩 Apply by submitting the Reduced Medical Rate Application and required docs to the Reservation Office.
