Reverse obliquity intertrochanteric femur fractures are notoriously difficult. Despite reasonable initial reduction and fixation (2), this one went onto nonunion and cutout (1). Templated for possible total hip replacement (3). Removed hardware and revised to restoration modular stem with bipolar head (4) and now the patient is ambulating and feeling much better on postop day 1.
