• MANAGEMENT and SUPERVISION- Our highly skilled care managers will conduct an in-home/facility consultation to assess each client's needs and collaborate with the registered nurse to formulate a comprehensive and personalized care plan to ensure the right services shall be provided. Care manager shall submit the customized care plan to the Director of Clinical Services for review, recommendation and approval.

• Service Quality- We select the home health professional based on the client's needs, preferences, personality, and schedule. Our care team will periodically visit the client to ensure the care provider is completing all assigned tasks. A Kind Heart management team will also communicate and share information with families, the facility and client’s physician about the client's care to ensure that your loved one receives the right care at the right time.
